Care Plans - GPCCMP
If you have a chronic or terminal medical condition that is expected to last six months or longer, you may be eligible for a GP Chronic Condition Management Plan (GPCCMP).
This plan allows you access to:
- Up to 5 Medicare-subsidised visits per calendar year to allied health providers of your choice (e.g. physiotherapists, dietitians, podiatrists, etc.).
How It Works
- Eligibility: You must have a chronic or terminal condition expected to be present for 6 months or more.
- Appointment Structure:
- A 15-minute appointment with a practice nurse, followed by
- A 15-minute appointment with your regular GP to finalise your plan and referrals.
Plan Reviews
- Your GPCCMP can be reviewed every 3 months to ensure your care plan remains current and continues to meet your needs.

Mental Health Treatment Plans
Mental Health Treatment Plans provide eligible patients to Medicare subsidised psychology and counselling services.
What to Expect
- A 30-minute appointment with your regular GP is required.
- This allows enough time to:
- Discuss your mental health concerns
- Complete the necessary paperwork and create your plan
- Organise any required referrals to mental health professionals

Cost
- These Health Assessments are usually bulk billed, meaning there is no out-of-pocket cost for eligible patients.
